An unannounced monitoring inspection was conducted on June 14, 2024 from approximately 10:00 am to 3:30 pm. There were 93 children in attendance and a total of 17 staff present. 6 staff records and 10 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed dancing, listening to a story, playing outside, playing in centers, eating lunch, and taking a nap, while two licensing inspectors were on-site.
Evacuation drills were conducted monthly with the most recent conducted on 05/28/24.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today,06/21/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 22.1-289.035-B-4 Description: Based on record review, and interview, the center failed to obtain a criminal background check and a central registry background check from any state in which the individual has resided in the preceding five years.
Evidence: The record for staff #2, date of employment 11/07/2023, indicated that they have lived in a state outside of Virginia in the preceding 5 years prior to employment. Interview with staff #2 confirmed that they lived in a state outside of Virginia in the preceding 5 years. Administration confirmed that the criminal background check was not requested prior to staff #2 beginning employment and a central registry background check was not requested no later than 30 days after beginning employment from that state.Plan of Correction: Not available online. Standard #: 8VAC20-780-130-A Description: Based on record review, and interview, the center failed to obtain documentation that each child has received the immunizations required before the child can attend the center.
Evidence:
1. The record for child #2, date of enrollment 06/03/2024, did not contain documentation of immunizations. Child #2 was in attendance. Administration confirmed that documentation of immunizations was not in the record.

Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Based on observation, the center failed to ensure that prepared infant formula shall be dated and labeled with the child's name.
