The licensing inspector conducted an unannounced complaint inspection on 06/25/2024 In response to a complaint that was received in the licensing office regarding allegations in the area of Staff Qualifications and Training and Supervision. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-E Complaint related: Yes Description: (REPEAT VIOLATION) Based on observation, the center did not ensure the operational responsibilities of the licensee by ensuring that the center's activities and services were in compliance with the standards and the center's own policies and procedures that are required by these standards.
1. On 06/05/2024 the center did not maintain compliance with their own policies and procedures required by these standards when the "Face t Name Transition Sheet," was not completed.
a. The center's supervision policy requires staff to complete a "Face-To-Name Transition sheet," to maintain compliance with the supervision requirements of the standards.
b. The Face-To-Name Transition Sheet dated 06/05/2024 did not capture when Staff B came into the twos classroom. The form showed 1 staff member with 15 two year-old children from 8:10am-9:00am.
b. The Face-To-Name Transition Sheet did not show any staff member in the two year-old classroom from 12:30pm - 1:40pm with 16 children during nap. Staff interviews confirmed Staff C was in the classroom, however a question mark was left after her name without record of the number of children present or when the staff came in or left the classroom.
c. The "F2N count" at the bottom of the Face-To-Name Transition Sheet was completed from 8:00am -10:00am and no other hourly, staff to child ratio counts were recorded.Plan of Correction: The following corrective action was provided by the center via email on 07/22/2024:
" Center Management will be conducting a face to name observation daily for the next 15 days to ensure the sheet is being completed according to LCG policy. There will also be a week-long re-training on LCG and VADOE Standards for Licensed Child Day Center regarding the two?s age group and ratio. This retraining will include expectations moving forward that staff will follow and complete all standards for the two?s classroom. "
