Technical Assistance provided regarding standard 22VAC40-131-180.J.1.c, driving records for applicants; documenting and addressing findings.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 06/04/2021 and concluded on 06/11/2021. The Program Director was contacted by email to initiate the inspection. The agency reported 26 children in care and 28 approved provider homes. The agency reports 0 new staff hired since the last inspection. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed 3 complete child record, 3 complete provider home record and 1 additional provider home records (background checks only). The inspector previously reviewed board officer records. Information gathered during the inspection determined a non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the agency.

Description:	Based on the review of the foster child record for foster child (FC) 3 and based on an interview with agency representatives (AR), the licensee failed to report a serious incident to the department?s licensing representative within 24 hours following the incident. Findings: 1.Based on a review of the record for FC-3, there was a serious incident documented on 03/31/2021. 2.Based on the review of the ?serious incident report? located in the record, the notifications section did not document the department as one of the entities that were notified of the incident 3.During the exit interview, AR1 and AR2 acknowledged that there was no report of the serious incident made to the department regarding the serious incident on 03/31/2021.
