Comments:

An unannounced monitoring inspection was conducted on August 20, 2019 from 10:55 AM - 3:15 PM. Family Life Services reports a total of 24 adoptive homes, 2 domestic agency adoption placements, and 6 cases in post-placement supervision. During this inspection, 9 provider home records (background checks only for six of the homes), 1 domestic agency adoption record and corresponding foster care record, and 2 post-placement supervision records were reviewed. One personnel was interviewed during the inspection. An exit interview was conducted at the conclusion of the inspection with the Director to review preliminary inspection findings. An acknowledgement form was signed. There was one violation. Upon the receipt of the violation notice, the licensee should identify the necessary corrective action and develop a plan of correction for each violation. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Standard #:	22VAC40-191-50-A-1-a
Description:	Violation: Based on a review of the record for an adoptive home (AH6) and interview with personnel (S1), the record for a provider (P1) contains a sworn statement that is not satisfactory. Evidence: The Sworn Statement for P1 from AH6 was not fully complete. Question #4 which reads "Have you ever been the subject of a founded complaint of child abuse or neglect outside the Commonwealth of Virginia" was not addressed. S1 reviewed the form and acknowledged that question #4 was not addressed.
Plan of Correction:	Director will get applicant to provide missing information on the Sworn Disclosure Form and will review with staff that we must check to make sure that the forms are filled out in their entirety.
