Comments:
An unannounced, monitoring inspection was conducted on January 10 , 2020 from 10:00 am to 2:30 pm at Jewish Family Services located at 6718 Patterson Avenue, Richmond, Virginia 23226. The agency reports 24 families approved and waiting for placement, and 3 families receiving post placement supervision services. The Adoption Social Worker was available for the inspection and present for the exit interview at the end of the inspection.

During this inspection, three adoptive home records were reviewed and six personnel records were reviewed. Also reviewed were background checks for all corporate officers and agents empowered to act on behalf of the corporation.

There was one citation for a violation of the Standards for Licensed Child-Placing Agencies. It was not a key health and safety standard. An acknowledgement of Inspection form was signed by the Adoption Social Worker on this date.

Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description: Violation: Based on a review of the personnel record for P-1 and an interview with an agency representative, the agency failed to complete an annual personnel evaluation.

Findings:
1) The most recent performance evaluation for P-1 was for the period covering 9-2017 through 9-2018.

2) The agency representative had an opportunity to review the file at the time of the inspection and acknowledged that the performance evaluation contained in the record was outdated.

Plan of Correction: The CEO will ensure her personnel evaluations are completed annually and maintained in her personnel record as required.
