Comments:

An unannounced, monitoring inspection was conducted on August 12, 2019 from 10:30 am to 4:00 pm at the Barker Foundation located at 2957 Monticello Drive, Falls Church, Virginia 22042. The agency reports 38 families approved and waiting for placement. The Domestic Program Director and the Domestic Adoption Support Specialist were available for the inspection and present for the exit interview at the end of the inspection. During this inspection, four adoptive home records were reviewed. Also reviewed were the background checks for five adoptive home files, approved since date of last inspection. Four personnel records were reviewed. Standards in Part II Organization and Administration and in Part IV Program Statement were reviewed during the inspection as well. There was one citation for a violation of the Standards for Child Placing Agencies. It was a key health and safety standard. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description:	Violation: Based on a review of the personnel record (P-4) and interviews with agency representatives (S-1 and S-2), the agency failed to obtain a search of the central registry before 30 days of employment at the agency ended. Evidence: 1) P-4 began employment in April of 2019. The central registry search results are dated 6/19/19. 2) S-1 and S-2 reviewed the record and agreed that the central registry search results were dated outside the 30 day timeframe.
Plan of Correction:	The Senior Director of Clinical and Support Services will ensure background check requests are submitted promptly.
