Comments:

An unannounced, monitoring inspection was conducted on January 29, 2020 from 10:30 am until 3:00 pm and on January 30, 2020 from 10:00 am until 3:00 pm at the Barker Foundation located at 2957 Monticello Drive, Falls Church, Virginia 22042. The agency reports 40 families approved and waiting for placement. The Senior Director of Clinical and Support Services and the Adoption Homestudy and Family Support Specialist were were available for the inspection. The Executive Director and the Adoption Homestudy and Family Support Specialist were present for the exit interview at the end of the second day. During this inspection, four adoptive home records were reviewed. Also reviewed were the background checks for eight adoptive home files, approved since the date of the last inspection. Six personnel records were reviewed. There were 2 citations for violations of the Standards for Child Placing Agencies. They were both key health and safety standards. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description:	Violation: Based on a review of the adoptive home record for (AH-1) and an interview with an agency representative, the agency failed to obtain medical examinations for the adoptive parents that contained written examiner comments addressing their mental and physical health in relation to their ability to care for a child. Evidence: 1) The medical examinations for the adoptive parents (AH-1) dated 12/03/18 and 12/14/18 do not include written examiner comments that address the applicants physical and mental health in relation to their ability to care for a child as required. 2) The agency representative reviewed the medical evaluations contained in the record and agreed that this section was left blank.

Description:	Violation:Based on a review of the adoptive home file for (AH-3) and an interview with an agency representative, the agency failed to obtain a search of the child abuse and neglect registry in another state for the other adult (OHM) residing in the home. Evidence: 1) The file for (AH-3) documents that another adult (OHM) resides in the home with the adoptive applicants. The sworn statement for OHM documents that the OHM lived in another state within the last 5 years. There was no central registry search from the other state contained in the record. 2) The agency representative reviewed the file at the time of the inspection and acknowledged that this was an oversight.
