Comments:

A monitoring inspection was initiated on 5/22/2024 and a Hybrid Inspection form emailed to agency and review of documentation was concluded on 5/23/24. The Executive Director was contacted by telephone to initiate the inspection. The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed documentation from (2) staff records: (offer letter/date of employment, application, resume, orientation checklist, trainings, education, job descriptions, references, background results, sworn statements, central registry results and performance evaluation. to ensure documentation was complete. An exit interview was conducted with the CEO and Executive Director on 6/7/2024 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Number of staff records reviewed (2) This office is a location for child-placing activities to occur at this location, but no active client and family records are currently housed here. There are no birth parent/child or provider home case records kept at this location, although the office is available to conduct any needed child placing activities. HR records are housed at this location. The agency has hired (2) new staff since the last inspection. An exit meeting was conducted via TEAMS with CEO and Executive Director of Adoption Services on 6/7/2024. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law.
