Comments:
An unannounced monitoring inspection was completed on 7/17/2019 from 10:05 a.m. to 2:45 p.m. The licensee reports a total of 20 children in foster care and 23 approved provider homes. No new staff were hired since the last inspection. During this inspection, 2 children?s records and 3 provider home records were reviewed. The Quality Assurance Caseworker and a Foster Home Recruitment Caseworker were available for the inspection and participated in the exit interview. Two Caseworker's also participated in the exit interview. The exit interview was held from 2:00 p.m. to 2:45 p.m. and an Acknowledgment of Inspection form was completed and left with the licensee at the end of the inspection. There were two citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interview, the agency failed to include information about the child's assessment of his progress and services needed in a quarterly report for FC2.

Finding:

FC 2 is 12 years of age. The child's 3rd quarter report did not include information about his assessment of his progress and description of services needed. During the exit interview, the 3rd quarter report was viewed and discussed. Staff participating in the exit conference acknowledged the report did not contain this information.

Plan of Correction: The Executive Director will train the TFC workers to include information about the child's assessment of his progress and services needed in a quarterly report.

Description: Violation: Based on record review and interview, the agency failed to include criteria for achievement for each goal in the quarterly progress reviews for Foster Child (FC) 1 and FC 2. Findings: FC 1 was placed 4/2/2018. The child's 4th quarter Progress Review report included goals and description of the child's progress. No criteria for achievement were document in the quarterly report. FC2 was placed 9/11/2018. The child's 3rd quarter Progress Review report did not document criteria for achievement of each goal. The quarterly reports were viewed and discussed during the exit interview. Staff participating in the exit conference acknowledged the quarterly reports did not contain criteria for achievement.

Plan of Correction: The Executive Director will train the TFC workers to include criteria for achievement for each goal in the treatment plan and quarterly progress reviews for foster children.
