Comments:
A monitoring inspection was initiated on 7/12/2021and concluded on 7/16/2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Direct reported that the current census was 14. The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two foster child records, three staff records, two provider home records, caseload assignment list, and on call schedule submitted by the agency to ensure documentation was complete. An exit interview was conducted with Executive Director on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable
standards or law, and violations were documented on the violation notice issued to the
facility.

Violations:

Description: Violation

Based on record review and interview, the licensee failed to maintain documentation of all prescribed medication in the record of foster child 2 (FC2).

Findings:

1) At intake, the custodial agency for FC2 reported the child was not known to be taking medications.
2) The case record for FC2 contained a written report documenting the child had a physical examination eight days following the child?s placement with the agency. The report included a statement from the physician indicating the child had historically had symptoms of a disorder and needed her medication.
3) The case record for FC2 contained a medication log documented the child was prescribed medications 65 days following placement. One medication was prescribed because the child was currently experience the symptoms that had also occurred historically.
4) Documents reviewed did not document effort to obtain the medications on the date of the physical examination and the date the medications were listed as prescribed per the medication log.
5) The findings were discussed during the exit interview and staff were interviewed. Staff 3 (S3) explained that while the child was not reported to be taking medication at intake, the agency later learned of a prescribed medication and were able to obtain the medication the birth parent already had for the child and that medication was available to the child prior to the prescription date listed 65 days after the child?s placement
6) Staff acknowledged the original medication was not listed on the child?s medication list.
