Technical Assistance:

Technical assistance was provided regarding the use of white out on official documents and ensuring that the licensee?s own application for placement forms are completed in their entirety by the local department of social services agencies.

Comments:

A monitoring inspection was initiated on 2/15/2022 and concluded on 2/16/2022. The Executive Director and the Quality Assurance Manager was available at the location at the time of the inspection. The inspector made an onsite inspection to the physical location and to review record storage keeping. COVID-19 questions asked and protocol measures were in place and followed by the licensee. The license was posted and the most recent violation notice was available for review. The inspector initiated the inspection. The Executive Director discussed that there were no agency or program changes during the entrance conference. The Licensing Inspector discussed the current census and requested items required to complete during the documentation review portion of the inspection. The Executive Director reported a current census of Twelve (12) foster children and Twenty-Five (25) foster homes. The Licensee had three (3) new foster homes since the last inspection and no new staff members since the last inspection. The Licensing Inspector reviewed three (3) newly approved foster care and Two (2) foster children. An exit interview was conducted with the Executive Director, Quality Assurance Manager, and the HR Director on the date of the inspection, where findings were reviewed and an opportunity was given for questions by the Executive Director and the Quality Assurance Manager. The opportunity was given to the Executive Director to provide any information or documentation which was not available during the inspection. The information gathered during the inspection determined that were no non-compliance(s) with applicable standards or law, and there were no violations documented on the violation notice issued to the facility
