Comments:
An onsite monitoring inspection was initiated on 3/7/2022 and concluded on 3/10/2022. The Licensing Inspector made an onsite visit to the Northern, Virginia location. The inspector discussed the Covid-19 protocol with the Acting Program Director present prior to proceeding with the inspection. After the protocol was cleared, the Licensing Inspector initiated the inspection. The Licensing Inspector met with the Program Manager and the HR Director at the location. The Licensing Inspector conducted an entrance conference regarding if there were any program or policy and procedure changes since the last inspection, and there were none reported. The Licensing Specialist discussed the previous violation report and the actions taken. The Licensing Inspector discussed the items required to complete the inspection and requested documentation for the review. The Acting Program Director reported a current census of Twelve (12) foster children in care and Twenty-Six (26) provider homes. The inspector reviewed Three (3) foster home provider records, One (1) foster home closed record, and One (1) foster child record, One (1) foster child discharge record, and Five (5) personnel records. An exit interview was conducted with the Acting Director on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. There were 3 violations of the Standards for Licensed Child Placing Agencies. The Acting Program Director was available at the exit interview to answer any questions.

Standard #: 22VAC40-131-110
Description: Violation:
Based on review of the record for FH1 and FH2 and an interview with the Acting Program Director, the licensee failed to ensure all materials and information received by the licensee indicated the date received.
Findings:
The record for FH1 did not include a date received on the copy of the applicants insurance policy received by the licensee. FH2 health statement documents received by the licensee did not include the dates the documents were received.

Standard #: 22VAC40-131-160-B-5
Description: Violation:
Based on review of the personnel records for P3 and P5 and an interview with the Acting Program Director, the licensee failed to ensure that each staff person had completed annual performance evaluations.
Findings:
The personnel documentation reviewed for P3 and P5 did not contain an annual performance evaluations. During an interview with the Acting Program Director, it was acknowledged that the personnel records failed to document the annual performance evaluations.

Description: Violation:
Based on review of the record for FH2 and an interview with the Acting Program Director, the licensee failed to ensure that the documentation complies with the standards for the home as required by 22VAC40-131-190.
Findings:
The home study documentation completed for FH2 referred to the applicant having a firearm, but did not address where the ammunition was stored for the firearm. Additionally, the home study documentation completed for FH2 did not address that the applicant had a pet.
