Technical Assistance:

The Licensing Inspector provided technical assistance regarding ensuring goals and objectives are measurable in the service plan and to ensure that all posted inspections does not contain the supplemental page with the protected health information attached

Comments:

Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 8/8/2022 from 12:45p.m. to 4:30 p.m. 8/9/2022 from 9:30 to 3:30 p.m. and 8/10/2022 from 9:30 to 12:30. The Licensing Inspector met with the Acting Director of Placement Services at the site to start the inspection. Number of approved provider homes: (17) Number of children in care: (7) The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (1) and (1) discharge record. Number of provider home records reviewed: (2) and (1) closed provider home. Number of staff records reviewed: (11) and (4) board officers reviewed during this inspection. Observations by licensing inspector: posting of the current license. An exit meeting was conducted on the date of inspection. There were no changes made to the policies or procedures since the last inspection. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law. The Licensing Inspector did provide technical assistance regarding ensuring goals and objectives are measurable in the service plan and to ensure that all posted inspections does not contain the supplemental page with protected health information attached.
