Comments:
An unannounced renewal inspection was initiated on 9/24/2019 from 9:40 a.m. to 2:45 p.m. at the agency?s Christiansburg office due to utility disruption at the Roanoke location. Foster child and foster home records were reviewed on this date. The inspection concluded on 9/25/2019 from 9:30 a.m. to 1:20 p.m. with review of personnel records at the Roanoke office. The licensee reports 33 children in foster care and 26 approved foster homes. 27 staff members serve this location. During this inspection three foster child and four foster home records were reviewed. 10 personnel records were reviewed on 9/25/2019. An additional six personnel records were reviewed during the licensing period.

Background checks for all Board Officers/Agents were reviewed on 8/26/2019 during a renewal inspection for the agency?s Abingdon office.

On 9/24/2019 the Director of Foster Care and Independent Living participated in the entrance conference and was available during the inspection. An Administrative Assistant also participated in the entrance conference.

On 9/25/2019 the Human Resource Administrator participated in the entrance conference, was available during the inspection, and was interviewed. The Site Leader/Foster Care Supervisor and Foster Care Supervisor were available during the inspection and participated in the exit interview. The Director of Foster Care and Independent Living participated in the exit interview by phone. An acknowledgement of inspection was completed and left with the agency on this date.

There were two citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on record review and interview, the agency failed to address all required components in the home study for foster home 1 (FH1).

Findings:
The home study for FH1 did not include information to confirm pets in the home were safe to be around children and would not present health a health hazard. Information gathered and documented in other forms noted pets were present in the home and vaccinations and licenses were verified by the Foster Home Specialist.

Plan of Correction: Program Director will remind staff to ensure all standards are addressed in the home study.

Description: Violation:

Based on record review and interview, the agency failed to obtain and document results of a criminal history background check within 3 years of the previous check for a staff member (P8).

Findings:

Results from a new criminal history search were due by 12/1/2018 for P8. The personnel record contained a new sworn statement and central registry check results but did not contain the results from a new criminal history search. The finding was discussed with agency representatives during the exit interview. Agency representatives reviewed the file and historical emails to determine if the results had been received. Results from a new search were not located. This staff member is not responsible for case management activities with children or families.

Plan of Correction: A criminal history search was resubmitted for the staff member and filed in the personnel record on 9/26/2019.

Following an HR transition, a new system has been established for regular review of personnel files, to include background checks, for all staff in the Child and Family Services division.
