Comments:
An unannounced, monitoring inspection was conducted on November 14, 2019 from approximately 9:40 am to 2:25 pm at C2Adopt located at 8100 Three Chopt Road, Suite 101, Henrico, Virginia 23229. The agency reports twenty adoptive families approved and waiting for placements and five children in post-placement supervision. The Executive Director was available by telephone during the inspection and participated in the exit interview via telephone which commenced at 2:15 pm on November 14, 2019.

During this inspection, three adoptive home records were reviewed. Background checks for three additional adoptive homes were reviewed. There were three citations for violations of the Standards for Child Placing Agencies. An Acknowledgement Form was signed by the social worker present for the inspection. Please see the violation notice on the Department's web site for violation of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on a review of the record for AH3, the licensee failed to obtain a signed authorization allowing the previous child placing agency to release information to the licensee about child caring performance of the providers.

Findings:
AH3 was approved by the licensee on 8/29/19. The home study documents that AH3 was previously approved by another child placing agency and had at least one placement. The record failed to document a release to exchange information and information regarding AH3's previous child caring performance as an approved provider.

During an interview with the social worker, it was acknowledged that a release of information was not obtained and information regarding previous child-caring performance as an approved provider was not received.

Plan of Correction: Supervisor will review with all workers the procedure for verify previous HS info
and obtaining releases.

Description: Violation:
Based on a review of the record for AH1 and interview with the executive director, the licensee failed to include the dated signature of the individual who completed the home study.


Findings:
AH1 was approved on 8/2/19. The home study documented in the record failed to include the signature of the individual who completed the home study.

During an interview with the executive director, it was acknowledged that she had not signed the home study she completed.

Description: Violation:
Based on a review of the Adoptive Home (AH) record and interview with the executive director, the agency failed to ensure the certificate of approval included the signatures and titles of the individuals approving the home.

Findings:
AH1 was approved on 8/2/19. The certificate of approval in the record failed to document the signatures and titles of the individuals approving the home.

During an interview with the executive director, it was acknowledged that she had not signed the certificate of approval.

Plan of Correction: Signature will be added
