Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced renewal inspection was initiated on 05/27/2020 and concluded on 06/02/2020. The executive director was contacted by telephone to initiate the inspection. The agency reported 25 approved adoptive homes. The agency reported 2 new interns since the last inspection. The inspector emailed the executive director a list of items required to complete the inspection. The inspector reviewed 3 adoptive home records, 5 staff records and background checks for the 4 corporate board officers. The agency submitted their "Pandemic/Covid-19" policies.

An exit interview was conducted by telephone on 06/02/2020 at approximately 10:00 am with the executive director. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency.

There were 2 citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department's public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on review of documents provided for board officer 1 and an interview with the executive director, the licensee failed to ensure a search of the central registry and a criminal history record check was completed for board officer 1 before the end of 30 days after the change in board officers.

Findings:
Board officer 1 was appointed in 9/2019. Based on an interview with the executive director, a search of the central registry and criminal record check was not completed for board officer 1 upon her appointment as an officer.

Standard #: 22VAC40-191-40-D-6
Description: Violation:
Based on review of the record for Adoptive Home (AH) AH2 and interview with the executive director, the licensee failed to ensure the required central registry checks were not dated more than 90 days prior to approving AH2.

Findings:
AH2 was approved on 11/26/19. The central registry check for both providers in AH2 are dated 7/3/19. During an interview with the executive director it was acknowledged that the 90 day requirement for the central registry checks were overlooked.

Plan of Correction: The Executive Director met with the social worker to review the standard
to ensure that she understood the 90 day requirement. The Executive Director has also met with all home study assessment social workers employed by C2Adopt and discussed options of double checking all time related standards by use of a newly created form that all staff will be asked to complete at the time that the complete an assessment and before submitting to the Executive Director for signature.

C2Adopt does exceed standards by asking clients to complete annual updates of their background checks.
