Comments:

An unannounced, mandated, monitoring inspection was conducted on March 9, 2022 from 9:30 am to 3:00 pm at the office of C2Adopt located in Henrico, Virginia. The agency reports twenty five adoptive families approved and waiting for placement and one child in post-placement supervision. The Executive Director was interviewed as part of the inspection process and was also available throughout the inspection to answer any questions. During this inspection, three adoption home records were reviewed. Background checks for 4 additional adoptive homes were reviewed. One personnel record and 3 intern records were reviewed. An exit interview was conducted with the Executive Director to discuss preliminary inspection findings on 3/9/22 at approximately 2:30 pm. An Acknowledgement Form was signed by the Executive Director on 3/9/22. There was one citation for violations of the Standards for Child Placing Agencies. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has ten calendar days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of the record for adoptive home (AH) AH1, the license failed to ensure background checks were completed before 3 years since the dates of the last background checks. Findings: AH1 was approved on 11/5/18. AH1 is a 2 provider home. The initial sworn disclosure statements for Provider (P) P1 and P2 are dated 7/17/18 and the most recent sworn disclosure statements for P1 and P2 are dated 8/25/21 which are beyond 3 years of 7/17/18. The initial central registry search results for P1 and P2 are dated 9/5/18 and the most recent central registry search results for P1 and P2 are dated 11/10/21 which are beyond 3 years of 9/5/18. The initial national criminal record checks for P1 and P2 are dated 8/7/18 and the most recent national criminal record checks are dated 10/21/21 which are beyond 3 years of 8/7/18.
