Comments:
An unannounced non-mandated monitoring inspection was conducted on October 10, 2019 from 10:30 AM - 2:05 PM. An exit interview was conducted at the conclusion of the inspection with the Director of Foster Care and Adoption - Western Region to review preliminary inspection findings. An acknowledgement form was signed. A follow-up phone interview was conducted on 10/21/2019 from 10:15 AM - 10:22 AM. An exit interview regarding this phone conversation was held on 10/22/2019 with the Director of Foster Care and Adoption - Western Region. This inspection was conducted as a result of an agency self-report. Hope Tree Family Services reports a total of 30 provider homes and 25 foster children in placement. During this inspection, one child's record and two provider home records were reviewed. Three personnel were interviewed. There was one violation.

The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description: Violation:
Based on review of the record for a foster child, FC1, and interview with personnel, S1, the agency failed to consult with the placing agency prior to the child's move from one foster home to another.

Evidence:
FC1 was moved from one foster home to another with notification of the move made to the placing agency two days later. The move was due to a placement disruption and did not meet the definition of short-term foster care. S1 self-reported the incident and acknowledged that the agency did not follow-up with the placing agency to consult with them about the decision prior to moving the child.

Plan of Correction: Supervisor will provide re-training regarding on-call procedures/responsibilities and STFC definition to all staff.
