This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An announced monitoring inspection was initiated on 7/8/2020 and concluded with an exit interview on 7/10/2020. The Executive Director was contacted by telephone to initiate the inspection. The agency reported 41 children in placement and 69 approved provider homes. The inspector emailed the Executive Director the list of cases for review for this inspection. The inspector reviewed five (5) child/youth records, 11 provider home records, and four (4) personnel records.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the agency.

Standard #: 22VAC40-131-290-L-2 Description: Violation:
Based on record review for Foster Home 6 (FH6), the agency failed to obtain a medical examination for Foster Parent 1 (FP1) that included all requirement information.
Findings:
The case record for FH6 contained medical exam reports for the foster parents. The medical exam report for FP1 did not include the date the exam was conducted. The findings were discussed during the exit interview. Staff 1 (S1) reviewed the exam report and acknowledged the findings.Plan of Correction: 1) Teaching Parent Supervisor will attempt to get a signed and dated physical from provider
2) Teaching Parent Coordinators and supervisor will double check medicals for provider signature and date
3) Teaching Parent Supervisor responsible is Melissa Blinn
