Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced renewal inspection was initiated on 3/29/2021 and concluded with an exit interview on 4/5/2021. The Executive Director was contacted by email to initiate the inspection. The agency reported 50 children in placement and 62 approved provider homes. The inspector reviewed five (5) child/youth records and ten (10) provider home records. Background checks for two (2) additional provider homes, two (2) personnel, and all board officers and agents were reviewed.
The executive Director responded to the notice of inspection and remained available during the inspection. The Executive Director, Human Resources Director, Treatment Foster Care Supervisor, Admissions Coordinator, and Teaching Parent Coordinator Supervisor participated in the exit interview.
Information gathered during the inspection determined non-compliance(s) with applicable standards. There was one (1) citation for a violation of the Standards for Licensed Child Placing Agencies.

See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on review of background check results for board officer 3 (B3), the licensee failed to have required background checks before three years since the dates of the last criminal history record check report.

Findings:
1. B3 began serving as a board officer on the board of directors 5/12/2017.
2. Criminal history record check reports for B3 are dated 4/14/2017 and 4/20/2020.
3. The criminal history record check was not conducted before three years since the date of the last criminal history record check.
4. The findings were discussed during the exit interview. S1 acknowledged the findings.

Plan of Correction: Executive Director has discussed with personnel responsible for obtaining background checks the need to start earlier, and involve the Executive Director immediately if the proper paperwork is not returned in a timely manner. This particular board member's file has been flagged for special attention, as he does not return paperwork in a timely manner.
