Comments:

A renewal inspection was initiated on 10/18/2021 and concluded on 10/22/2021. The Executive Director was contacted by telephone to initiate the inspection. The inspector emailed the Director and Associate Director a list of items required to complete the remote documentation review portion of the inspection. The Executive Director reported a current census of Forty (40) foster children in care and Fifty-Nine (59) provider homes. The inspector reviewed five (5) foster home provider records, three (3) additional foster home records-background checks only, seven (7) foster children records, including one (1) discharge foster child record, six (6) staff records. An exit interview was conducted with the Executive Director, Program Supervisor and Admissions on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined that there were no non-compliance(s) with applicable standards or law, and there were no violations documented on the violation notice issued to the facility.
