Comments:

A renewal inspection was conducted on 5/9/2022 from 11:45 a.m. to 4:00 p.m. and 5/10/2022 from 9:15 to 4:15 p.m. The Regional Director and Office Manager participated in the entrance conference. The inspector conducted an entrance conference regarding if there were any program or policy and procedural changes since the last inspection, and there were none reported. The Licensing Inspector reviewed the posted license and the previous inspection. The Licensing Inspector discussed the renewal application and noted if there were any changes. The Licensing Inspector reviewed storage keeping for case and personnel files. The licensee reported a census of 43 provider homes and 23 children in care. The Licensing Inspector reviewed Four (4) foster children in placement and One (1) discharge record. The Licensing Inspector reviewed Five (5) provider homes, One (1) suspended provider home and the background checks of Two (2) provider homes. During this review, all board officers were reviewed, and Seven (7) personnel records. An exit interview was conducted with the Regional Director for the Staunton location, Human Resources Director, and the Admissions Coordinator on the date of the inspection. Findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
