Comments:
An unannounced renewal inspection was completed on July 15, 2019 from 10:00 am to 3:00 pm at Cradle of Hope located at 1934 Old Gallows Road, Suite 350, Tysons Corner, Virginia 22182. Currently, there are ten approved provider homes waiting for adoptive placement and three children in post placement supervision. During this inspection, three adoptive home records and five personnel records were reviewed. In compliance with licensing requirements for renewal application, all documentation for corporate board officers and agents of the corporation were reviewed, including required background checks. For the purpose of the violation notice, "AH" refers to adoptive home and "P" refers to personnel. There were two citations for violations of the Standards for Child Placing Agencies. One was a key health and safety standard. The Homestudy Coordinator was available for the inspection and present for the exit interview at the end of the inspection. An Acknowledgement of Inspection Form was signed by the Homestudy Coordinator on this date. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-150-B
Description: Violation: Based on a review of the personnel records (P-1, P-2 and P-3) and an interview with an agency representative, the agency failed to provide an initial orientation for staff who work with children and families or supervise staff that work with children and families. Evidence: 1) There was no documentation in the personnel files of P-1, P-2, or P-3 that they had received an initial orientation that addresses elements as required under 22VAC40-131-150.B.1, B.3, and B.4. prior to assuming the responsibilities of their positions. 2) The agency representative reported that the orientation was provided but acknowledged that there was no way to verify that it had been completed.

Plan of Correction: Initial orientation to be provided for these three social workers and documented in their personnel files. New personnel file checklists will ensure future hires receive initial orientations prior to assuming the responsibilities of their positions and that their training is documented in their personnel files.

Description: Violation: Based on a review of the personnel records (P-2 and P-4) and an interview with an agency representative, the agency failed to provide pre-service training for adoptive and foster parents to professional staff. Evidence: 1) There was no documentation in the personnel files of P-2 and P-4 that they had participated in the agency's pre-service training for adoptive and foster parents. 2) The agency representative reported that the training was provided but acknowledged that there was no way to verify that it had been completed.

Plan of Correction: New personnel file checklists will ensure that these social workers and future hires complete the preservice training required of adoptive/foster parents within one year of their employment at CHAC and that their training is documented in their personnel files.
