Comments:
An unannounced monitoring inspection was completed on January 23, 2020 from 10:00 am to 2:00 pm at Cradle of Hope located at 1934 Old Gallows Road, Suite 350, Tysons Corner, Virginia 22182. Currently, there are ten approved provider homes waiting for adoptive placement The agency currently has 16 families approved and waiting for placement. Twelve adoptive homes have been approved since the last inspection. During this inspection, three adoptive home records were reviewed in their entirety. Background check results were reviewed in the remaining nine records. Five personnel records were also reviewed.

There was one citation for a violation of the Standards for Child Placing Agencies. It was a key health and safety standard. The Homestudy Coordinator was available for the inspection and present for the exit interview at the end of the inspection. An Acknowledgement of Inspection Form was signed by the Homestudy Coordinator on this date.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-180-J-1
Description: Violation: Based on a review of the adoptive home record for (AH-3) and an interview with an agency representative, the agency failed to obtain a driving record for the adoptive father as required.

Evidence:
1) AH-3 was approved as an adoptive provider. The record did not contain a driving record for the adoptive father.

2) The agency representative acknowledged that this was an oversight.

Plan of Correction: The social worker will obtain a driving record for the adoptive father immediately. Social workers will review the paperwork that is submitted by adoptive families carefully to ensure they are compliant with the requirements of the regulations.
