22VAC40-131 PROVIDER HOMES 22VAC40-131 CHILDREN'S SERVICES 63.2 General Provisions.

Comments:

As a result of self-reports, an unannounced focused inspection was conducted on January 22, 2020 from 9:00 am to 12:45 pm at KidsPeace located in Chesterfield, VA. During this inspection, One (1) foster home record and One (1) foster child record was reviewed. Policy and Procedures were reviewed. One agency representative was interviewed during this inspection. An exit meeting was conducted with the Executive Director on January 22, 2020 at 12:15 pm. An acknowledgment form was signed and left at this location. There were two (2) citations for violations of the Standards for Child Placing Agencies. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days.

Standard #:	22VAC40-131-250-M
Description:	Based on the review of the record for Foster Child (FC)1 and based on an interview with an agency representative (AR), the licensee failed to arrange a pre-placement visit for the child in the prospective home. Findings: 1. Based on the documentation located in the record for FC1, there was no documentation of a pre-placement visit for FC1 in the prospective home and no documented reason why the pre-placement visit did not take place. 2. During an interview, AR1 acknowledged that a pre-placment visit in the prospective home did not take place.
Plan of Correction:	Although a pre-placement visit occurred, it did not occur in the prospective home. State Manager will ensure that all staff are aware of the full regulation in that not only a pre-placement visit occurring but it also occurring in the prospective home
Standard #:	22VAC40-131-320-E
Description:	Based on an interview with an agency representative (AR), based on the review of the record for Foster Child (FC) 1 and based on the review of a self-reported incident that was submitted to the department on 01/03/2020 for an incident occurring on 12/29/2019, the licensee failed to report a serious incident to the department's licensing representative within 24 hours following the incident. Findings: 1. During an interview, AR1 acknowledged that the incident was not reported within 24 hours. 2. Based on the review of the record for FC1, the incident occurred on 12/29/2019 and was not reported until 01/13/2020.
Plan of Correction:	State Manager will ensure that all staff are aware of this standard and that when she is not in the office that they are aware that they must report the serious incidents to appropriate channels.
