Technical Assistance:
Technical assistance provided regarding standard 22VAC40-131-140.E, discussed position titles; the licensee presented evidence of compliance regarding a new hire and the position of program supervisor.

Comments:
An unannounced mandated renewal inspection was conducted to KidsPeace located at 7631 Hull Street Road, Chesterfield, VA 23235 on February 24, 2020 and from 9:00 am to 4:00 pm and on February 25, 2020 from 9:00 am to 1:00 pm. KidsPeace continues to provide foster care, short-term foster care and treatment foster care services. The agency reports forty-two (42) children in care and twenty-eight (28) approved foster homes. There was 1 new staff hired for this location since the last monitoring inspection. There were no reported board member changes.

During the inspection, the following activities transpired:
1. Three (3) complete foster home records were reviewed
2. Four (4) complete foster child records were reviewed
4. One (1) additional foster child record was reviewed (discharge records only)
5. Interviews with agency representatives were conducted
6. Board Member background records were reviewed
7. Two (2) personnel files were reviewed
8. The renewal application with all required attachments was reviewed

There were three (3) citations for violation of the Standards for Child Placing Agencies. An exit meeting was conducted with agency representatives (Program Supervisor and Senior Family Consultant) on February 25, 2020 at 12:30 p.m. to discuss the inspection findings. Upon receipt of the violation notice, the licensee should develop a plan of correction for each violation. The plan of correction should include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Based on the review of the record for FH2 and based on an interview with an agency representative, the licensee failed to document the signature of the person completing the home study as well as failed to document the signature of the executive director or designee on the home study for FH2.

Findings:
1.The home study located in the record for FH2 did not contain a signature of the executive director and contained only the written name of the person completing the home study without this person?s signature.
2.During the exit interview, AR1 reviewed the copy of the home study that was obtained from the record and confirmed the absence of the signature for the executive director or designee. AR1 confirmed the typed name of the person completing the home study.

Plan of Correction: State Manager will ensure that the Home Study Supervisor is signing all home studies. State Manager will ensure that a signature spot is added to the home study for the person completing the home study.

Based on the review of the record for Foster Home (FH) 1 and FH2 and based on an interview with an agency representative, the licensee failed to consider all information gathered when assessing the applicant?s capabilities to care for children.

Findings:
1.The record for FH1 and FH2 contained Virginia Department of Motor Vehicle Transcript of Driver History Records, documenting convictions and accidents. Based on the review of the records, there was no documentation in the record that the licensee considered these factors when assessing the applicant?s capabilities to care for children.
2.During the exit interview, AR1 reviewed the Virginia Department of Motor Vehicle Transcript of Driver History Records and reviewed the provider home records, including the home study reports. During this interview, AR1 acknowledged that this information was not documented as being considered when they assessed the applicant?s capabilities to care for children.

Plan of Correction: State Manager will readdress this licensing standard with FRS completing home studies and make sure they are aware that assessing all information received should be discussed in the home study

Based on the review of the record for Foster Child (FC) 4 and based on an interview with agency representatives (AR), the licensee failed to document the involvement of the birth parents in developing the child?s individualized service plan and quarterly progress reports and in updating the service plan for FC4.

Findings:
1.The record for FC4 did not contain documentation of the involvement of the birth parents in developing the child?s individualized service plan and quarterly progress reports and in updating the service plan for FC4.
2.During the exit interview, AR1 and AR2 reviewed the record for FC4 and determined that at the time of the inspection that the record for FC4 did not contain documentation of the involvement of the birth parents in developing the child?s individualized service plan and quarterly progress reports and in updating the service plan for FC4.

Plan of Correction: State Manager will retrain the Family Consultants on this standard and will ensure that this is completed on all future individualized service plans and quarterly progress reports.
