Technical Assistance:

22VAC40-131-260 & 180: Based on the current formats used the Foster Provider Home Study and Child Social History for the records reviewed provided limited information; the licensee is encouraged to add more details or adjust the format used.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on November 5, 2020 and concluded on November 12, 2020. The Program Director was contacted by telephone to initiate the inspection. The agency reported 38 children in care, 26 approved foster provider homes and no newly hired employee. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed 4 complete child records, 3 complete foster home provider records and 1 additional foster home provider record-background checks only submitted by the agency to ensure documentation was complete. Board Officers were previously reviewed. Information gathered during the inspection-determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on the review of the Foster Children records presented for Foster Child (FC) 1, 2, and 3, the licensee failed to complete a social history on each child within 45 days of the date of the child?s placement. Findings: 1.The social history document, titled ?bio-psychosocial addendum? for FC 1, 2, and 3 was signed and dated 11/06/2020, more than 45 days of placement. 2.Upon interview, AR-1 reported that the addendum contains required elements of the social history necessary to meet the regulatory requirements for social history. 3.Upon interview, AR-1 acknowledged that the addendums were beyond 45-days of the child?s placement.

Description:	Based on the review of the file for Foster Home (FH)-3 and based on an interview with an agency representative (AR), the licensee failed to obtain a tuberculosis screening or test results that contained the screening or test date and the results that was no older than 13 months at the time of the home approval. Findings: 1.A ?Tuberculosis Screening Questionnaire? signed by a ?Healthcare Provider NP/MD on 11/17/2018 was presented in the records for review for FH-3. 2.There were no additional tuberculosis screenings for the foster parent presented for review for FH-3. 3.Upon interview on 11/12/20, AR-1 stated that FH-1 there were no additional tuberculosis screenings to present for FH-3 and acknowledged that the screening presented was older than 13 months at the time of approval.
