Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on February 17, 2021 and concluded on February 26, 2021. The Program Director was contacted by email to initiate the inspection. The agency reported 36 children in care, 24 approved foster provider homes and no newly hired employee. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed 4 complete child records, 3 complete foster home provider records, 3 employee records and board officer records submitted by the agency to ensure documentation was complete.
Information gathered during the inspection-determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-200-G
Description: Based on the review of the information presented for Foster Home (FH)-3 and based on an interview with an Agency Representative (AR), the licensee failed to issue a certificate of approval to the provider and maintain a copy of the certificate I the provider?s file.

Findings:
(1)The records presented did not contain documentation of a certificate of approval for both foster parents approved for FH-3.
(2)During the exit interview, AR-1 acknowledged the absence of the certificate of approval for both foster parents approved for FH-3.

Plan of Correction: State Manager will review requirement with FRS in regards to updating COA when changes are made. FRS will add a new COA into the provider home for this instance.

Standard #: 22VAC40-131-230-E-2
Description: Based on the review of the information presented for Foster Home (FH)-2 and based on an interview with an Agency Representative (AR), the licensee failed to review the medial information the licensee used to make the last approval decision.

Findings:
(1)The records presented did not contain documentation of the licensee?s review of the medical information used to make the last approval.
(2)During the exit interview, AR-1 acknowledged that medical information used to make the last approval was no reviewed at the time of the re-evaluation.

Plan of Correction: State Manager will work with FRS to ensure that medical evaluations are redone ever re-approval and/or comment on the medical stability ? no changes of physical health for provider homes
