Comments:

An unannounced monitoring inspection was conducted on February 08, 2022 from 9:45 am to 4:30 pm and February 09, 2022 from 8:45am-1:00pm at the Chesterfield location. The licensee reports a total of twenty-eight (28) foster children in care at this location. The licensee reports a total of twenty-one (21) active foster homes for this location. There was one (1) new staff hired since the last inspection. The following is a list of activities for this inspection: 1.Two (2) foster children files reviewed 2.Three (3) complete foster home records reviewed 3.Two (2) staff records reviewed 4.Inspection of the physical plant completed Staff were available for the exit interview which commenced at approximately 12:30 p. m. on February 09, 2022 at the Chesterfield location. An acknowledgement form was signed and left with the licensee at this time. There were two (2) citations for violation of Standards for Licensed Child-Placing Agencies. Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non- compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on the review of the Foster Home (FH) record for FH-1 and based on an interview with an Agency Representative (AR), at the time of re-evaluation, the licensee failed to obtain new signed agreements from the provider covering elements required for maintaining confidentiality of information and prohibition of the use of corporal punishment. 1.The record did not contain documentation of new signed agreements from the provider covering elements required for maintaining confidentiality of information and prohibition of the use of corporal punishment at the time of re-evaluation. 2.During an exit interview, AR1 acknowledged the absence of new signed agreements from the provider covering elements required for maintaining confidentiality of information and prohibition of the use of corporal punishment at the time of re-evaluation.

Description:	Based on the review of the record for Foster Child (FC)-2 and based on an interview with an Agency Representative (AR), the licensee failed to arrange a preplacement visit for the child in the prospective home. Findings: 1.The record documented that there was no pre-placement visit in the prospective home due to scheduling conflicts. 2.During the exit interview, AR-1 acknowledged that the pre-placement visit was not conducted in the prospective home.
