Comments:

An unannounced monitoring inspection was conducted on November 21, 2019 from 10:10 a.m. to 2:25 p.m. The agency continues to provide independent living arrangements (ILA). At the time of the inspection the agency reports fourteen youth placed in ILA. During this inspection, two personnel records and two youth records were reviewed. Staff S3 was available to respond to questions during the inspection. An exit meeting was conducted with Staff S3 on November 21, 2019 to discuss preliminary inspection findings. An acknowledgment form was signed.
