Comments:
An unannounced, mandated, renewal inspection was conducted on 10/04/2019 at the agency's new location, 950 N. Washington Street, Suite 227 Alexandria, Virginia 22314. The agency is currently providing services to four foster children and three foster families. At this inspection, one foster home record was reviewed, one foster child's record was reviewed and one personnel record were reviewed. In compliance with licensing requirements for a renewal application, all documentation for corporate board officers and agents of the corporation were reviewed, including required background checks. The Program Director and the Case Manager were available for the inspection and the exit interview at the end of the day. There were two citations for violations of the Standards for Licensed Child-Placing Agencies. They were key health and safety standards.

The on site inspection began at 10:30 am and ended at 1:30 p.m. The Acknowledgement of Inspection Conducted and Licensee Responsibility form was signed by the Program Director on 10/04/2019.

Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Violations:
Standard #: 22VAC40-131-280-C-1
Description: Based on a review of the foster home record (FH-1) and an interview with agency representatives, the agency failed to include the code of ethics and mutual responsibilities form in the foster home agreement for the child (FC-1) currently placed in their home.

Evidence:
1) The foster home agreement for FC-1 contained in the record of FH-1 did not include the language required in the code of ethics and mutual responsibilities form.
2) The agency representatives looked in the file and agreed that the code of ethics form was missing.

Plan of Correction: RCI case managers will include the code of ethics and mutual responsibilities form in the foster home agreement for children currently placed by recreating a check list of all forms that will be reviewed monthly to ensure compliance.

Description: Based on a review of the child's file (FC-1) and an interview with agency representatives, the agency failed to interview the child privately at least one time each month.

Evidence:
1) The home visit contact notes in the file for FC-1 for August and September of 2019 do not document that the child was interviewed privately.
2) The agency representatives looked through the file and agreed that private interviews did not take place in August and September of 2019 as required.

Plan of Correction: RCI case managers will have a 1:1 private visit on a monthly basis with each child in placement and document in the foster child file with the 1:1 symbol in the progress note section of the file after the case manager signature.
