Comments:
A renewal inspection was initiated on 9/07/2021 and concluded on 9/10/2021. The Program Director and the Child-Placing Supervisor/Case Manager were contacted by telephone to initiate the inspection. The Program Director reported that the current census was four (4) children. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster home records, two (2) foster children's records, and two (2) staff records. Background checks for two corporate officers were also reviewed in compliance with regulations regarding renewal applications. An exit interview was conducted with the Program Director and the Child Placing Supervisor/Case Manager on 9/10/2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the agency.

Description: Violation:
Based on a review of the "Intake Assessments" for two children, (FC-1 and FC-2) and an interview with agency representatives (AR-1 and AR-2), the agency failed to include the date of planned achievement for the children's permanency planning goals.

Evidence:
1) The "Intake Assessment" for FC-1 and FC-2 each identified permanency planning goals but the sections regarding the dates of planned achievement were left blank.
2) In an exit interview with agency representatives on 9/10/21, AR-1 and AR-2 acknowledged that the dates for planned achievement were not documented as required.

Description: Violation:
Based on a review of the "Intake Assessments" submitted for two children (FC-1 and FC-2) and an interview with agency representatives (AR-1 and AR-2), the agency failed to prepare the children for placement and arrange preplacement visits for the children in their prospective foster homes.

Evidence:
1) The "Intake Assessments" for FC-1 and FC-2 documented that no preplacement visits took place because their placements were emergency placements. FC-1 was placed on 7/27/21 and FC-2 was placed on 6/03/21.
2) The "Intake Assessments" for both children documented that their placements were not emergency placements. FC-1 has been in care since 10/26/18 and FC-2 has been in care since 4/09/21.
3) In an exit interview with agency representatives on 9/10/21, AR-1 and AR-2 acknowledged that the placements of FC-1 and FC-2 did not meet the definition of an emergency placement
