Comments:
A non-mandated focused, monitoring inspection was initiated on 10/25/2021 and concluded on 10/29/2021. The Program Director and the Child-Placing Supervisor/Case Manager were contacted by telephone to initiate the inspection. The Program Director reported that the agency currently has 4 families approved and 5 children in placement. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster home records, two (2) children's records, and two (2) personnel records submitted by the agency to ensure documentation was complete. There were no changes to any polices or procedures reported. An exit interview was conducted with the Program Director and the Child-Placing Supervisor/Case Manager on 10/29/2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations were documented on the violation notice issued to the agency.

Violations:
Standard #: 22VAC40-131-260-B
Description: Violation: Based on a review of the Social History submitted by the agency for a foster child (FC-1) and an interview with an agency representative (AR-1), the agency failed to include required elements in the Social History Report.

Evidence:
1) The Social History Report for FC-1 dated 10/26/21 did not include:
a. 260.B.4 information on the child's relationships with family members and their involvement with the child.
b. 260.B.6 the addresses of providers of medical treatment
c. 260.B.10 emotional or psychological problems the child has experienced within the last 13 months.
2) In an exit interview on 10/29/21, AR-1 acknowledged that the information was not documented in the child's social history as required.

Description: Violation: Based on a review of the medical evaluation submitted by the agency for a foster child (FC-2) and an interview with an agency representative (AR-1), the agency failed to ensure that the child's annual medical examination included required components.

1) The medical examination for FC-1 dated 5/17/21 does not include the child's:
a. 290.C.3 Current physical condition
b. 290.C.4 Growth and development
c. 290.C.5 Visual acuity
d. 290.C.6 Auditory acuity
e. 290.C.7 Nutritional status
f. 290.C.8 Evidence of freedom from communicable disease, including tuberculosis
g. 290.C.9 Allergies, including food and medication allergies
h. 290.C.10 Chronic conditions
i. 290.C.11 Disabilities
j. 290.C.12 A copy of the record of immunizations the child has received since the last examination.
2. In an exit interview on 10/29/21, AR-1 acknowledged that the information was not included on the medical examination report as required.
