Type of inspection: Monitoring An on site inspection was conducted on 3/02/2023 from 10:00 am until 1:00 pm. Number of children in care: 1 Number of approved provider homes: 3 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 1 Number of staff records reviewed: 2 Number of interviews conducted with staff: 1 entrance interview and exit interview with the Executive/Program Director and the Child Placing Supervisor/Case worker. Observations by licensing inspector: The license was posted along with inspection documents. Additional Comments/Discussion: 3 provider home record was reviewed. An exit interview was conducted on the date of the inspection. The evidence gathered during the inspection determined non-compliance with applicable standards or laws, and violations were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violations will be addressed in order to return the facility to compliance and maintain future compliance with the applicable standard or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector?s immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Description:	Violation: Based on a review of a child's record (FC-1) and an interview with agency representatives, the agency failed to address required elements in the child's quarterly progress summary. Findings: 1) The quarterly reports for FC-1 dated 9/2/22 and 12/3/22 do not document the criteria for achievement for educational goals. 2) An Individualized Education Plan dated 9/23/20 for FC-1 contained in the record documents that FC-1 is in the 6th grade. 3) The criteria for achievement of FC-1's educational goal in the "Quarterly Summary Report" dated 9/3/22 documents that FC-1 will be promoted to the 7th grade in the 2021/2022 academic year. 4) The criteria for achievement of FC-1's educational goal in the "Quarterly Summary Report" dated 12/3/22 documents that FC-1 will be promoted to the 8th grade in the 2021/2022 academic year. 5) The "Quarterly Summary Report" dated 12/3/22 documents the projected discharge date as 4/19/22. 6) In an interview with agency representatives (AR-1 and AR-2) during the inspection to review preliminary findings, AR-2 acknowledged that the criteria for achievement and the projected discharge date were not updated.
