Type of inspection: Renewal
An on site inspection was conducted on 10/12/2023 from 10:00 am until 2:00 pm.

Number of children in care: 1
Number of approved provider homes: 3
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
Number of children?s records reviewed: 1
Number of staff records reviewed: 2
Number of interviews conducted with staff: 1 entrance interview and exit interview with the Executive/Program Director and the Child Placing Supervisor/Case worker.
Observations by licensing inspector: The license was posted along with inspection documents.
Additional Comments/Discussion: 3 provider home record were reviewed.
An exit interview was conducted on the date of the inspection.

The evidence gathered during the inspection determined non-compliances with applicable standards or laws, and violations were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violations will be addressed in order to return the facility to compliance and maintain future compliance with the applicable standard or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector?s immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that
serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department?s inspection findings are subject to public disclosure.

Description: Violation: Based on a review of the re-evaluation for a foster family (FH-1) and an interview with agency representatives (AR-1 and AR-2), the agency failed to include elements in the re-evaluation as required.

Evidence:
1) The re-evaluation for FH-1 did not include new signed agreements regarding confidentiality and corporal punishment.
2) At an exit interview on 10/12/23, AR-1 and AR-2 acknowledged that the information was not included in the re-evaluation as required.

Description: Violation: Based on a review of the agency placement agreement for a child (FC-1) and an interview with agency representatives (AR-1 and AR-2), the agency failed to secure written authority to make the placement prior to the child's placement.

Evidence:
1) The agency placement agreement for FC-1 was dated 8/03/23. The social history contained in the record documented that FC-1 was placed on 7/11/23.
2) The agency placement agreement for FC-1 dated 8/03/23 was not signed by the legal guardian.
3) At an exit interview on 10/12/23, AR-1 and AR-2 acknowledged that the placement agreement was not signed by the guardian and was dated after FC-1 was placed.

1) There was no written agreement with FH-1 for FC-1 contained in either record.
2) At an exit interview on 10/12/23, AR-1 and AR-2 acknowledged that the foster home agreement was not in the records as required.
