Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on February 11, 2021 and concluded on February 16, 2021. The Program Director was contacted to initiate the inspection. The agency reported 12 approved homes and 18 foster children in placement. There were no new staff since the last inspection. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed two (2) provider home records and two (2) foster child records to ensure documentation was complete. There are no new board officers. Board officers were reviewed on 03/11/2020 and information remains current. An exit interview to discuss findings was held with the Program Director at the conclusion of the inspection. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description:	Violation: Based on review of the record for a foster child (FC1) and interview with personnel (S1), the licensee failed to include a required element in the treatment foster child's individualized service plan. Evidence: 1. The individualized service plan for FC1 dated 10/14/2020 did not contain the anticipated target date for the child's discharge from the program. 2. During the exit interview, S1 acknowledged that the plan did not contain the discharge date.
