Comments:
An unannounced monitoring inspection was conducted on 12/15/2021 from 11:05 a.m. to 5:00 p.m. and 12/16/2021 from 10:25 a.m. to 3:00 p.m. The Licensing Inspector contacted the Program Director and conducted a COVID pre-screening prior to the inspection.

The agency reported 11 children in foster care and 12 approved provider homes. One new staff member was hired since the last inspection. During this inspection two (2) foster child, two (2) provider home, and two (2) personnel records were reviewed.

The Program Director participated in the entrance conference, office tour, remained available during the inspection, and was interviewed. The Foster Home Recruiter was interviewed. An exit interview was conducted with the Program Director on 12/16/2021 from 3:00 p.m. to 3:20 p.m.

There were three (3) citations for violations of the Standards for Child-Placing Agencies. Violations are documented on the Violation Notice.

Violations:
Standard #: 22VAC40-131-290-C
Description: Based on record review , the agency failed to obtain an annual written physical examination report for Foster Child 2 (FC2) addressing required elements.

Findings:
1. The case record for FC2 contained a Medical Statement completed in June 2021.
2. The Medical Statement did not address growth and development, nutrition, vision, or auditory acuity.
3. S1 acknowledged the findings.

Standard #: 22VAC40-131-290-K-1
Description: Based on record reviews, the agency failed to obtain a tuberculosis screening report containing required elements for Household member 1 (HH1) in Foster Home 1 (FH2).

Findings:
1. The records for FH2 contained tuberculosis screening documents for HH1.
2. The tuberculosis screening for HH1 documented a screening was conducted and the form was signed by medical professionals.
3. The medical professional did not include a statement that HH1 is free from tuberculosis in communicable form.
4. The findings were discussed during the exit interview. The Foster Home Recruiter, Staff 2 (S2), was interviewed and reviewed the document.
5. S1 and S2 acknowledged the findings.

Standard #: 22VAC40-131-350-B
Description: Violation

Based on record review and interview, the agency failed to address required elements in written quarterly progress reviews for Foster Child 1 and 2 (FC1 and FC2).

Findings:
1. Two written quarterly progress reviews for FC1 did not include a description of changed needed, the child?s assessment of his progress, a list of medications, or a projected discharge date.
2. One written quarterly progress review for FC2 did not include a projected discharge date.
3. Staff 1 (S1) was interviewed and contacted the caseworker to determine if additional information was captured regarding these elements. Additional information was not available by interview.
4. S1 acknowledged the findings.
