Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/12/2022 9:25 a.m. to 2:30 p.m.

An entrance conference was conducted with the Program Director. Draft inspection documents were provided to the Program Director and an exit interview was conducted with the Program Director on the date of inspection.

Number of children in care: 10
Number of approved provider homes: 7
The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Licensing posting, licensed capacity, and caseloads were inspected.

Number of staff records reviewed: 1

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Description: Violation:
Based on record review and interview, the agency failed to include required documents in the record for Staff 1 (S1).

Findings:
1. The record for S1 did not include references.
2. Staff 2 (S2) was interviewed. S2 contacted the agency?s Human Resource office. S2 informed the licensing specialists that the Human Resource office reported references were not obtained for S1.
3. The findings were discussed with S2 during the exit interview and S2 acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to include the date and summary of content information from references in the narrative home study for Provider Home 1 (PH1).

Findings:
1. The home study for PH1 did not include dates or summary of content information from references obtained.
2. The home study for PH1 stated PH1 ??provided Pressley Ridge with three references which can be found in their personnel file.?
3. The findings were discussed with Staff 2 (S2) during the exit interview.
4. S2 contacted Staff 3 (S3), the Recruitment and Certification Specialist.
5. S3 reviewed the home study and acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to, within 30 days of placement, place in the file for Child 1 (C1), a written assessment including required elements as required by 22VAC40-131-250.G.

Findings:
1. The written assessment in the file for C1 included a signature dated 57 days following the date the child was placed.
2. The findings were discussed with Staff 2 (S2) during the exit interview.
3. S2 acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to complete a Social History for Child 1 (C1) that addressed required elements.

Findings:
1. The record for C1 contained a document titled VA Social History. The document was blank.
2. The record for C1 contained a document titled Comprehensive Biopsychosocial Assessment that was partially completed
3. Staff 2 (S2) was interviewed. S2 reported the Comprehensive Biopsychosocial Assessment is now being used to address standard 22VAC40-131-260. B.
4. The Comprehensive Biopsychosocial Assessment for C1 did not address or include:
? The child?s permanency planning goal
? Services needed to achieve the child?s permanency goal
? Family structure, relationships, and involvement with the child
? The child?s developmental history
? Education and occupation of the child?s parents, siblings, aunts, uncles, and grandparents
? Medical and psychiatric history of the child?s parents, siblings, aunts, uncles and grandparents as it relates to the suitability for the child?s placement
5. The findings were discussed with Staff 2 (S2) during the exit interview.
6. S2 acknowledged the findings.
