Comments:

Type of inspection: Renewal An unannounced, mandated renewal inspection was conducted on 03/05/2024 from 10:45 a.m. to 3:40 p.m.. A preliminary findings review was conducted onsite post-inspection and an Acknowledgement of Inspection form was emailed to the Program Director for signature. Foster Children Census: 14 Number of provider homes: 7 Number of foster child records reviewed: 2 Number of provider homes reviewed: 1 Number of staff records reviewed: 3 Number of Board/Agent records reviewed: 6 Number of staff interviews: 2 The licensing inspector also included a tour of the physical space which included the office setting and conditions. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Description:	Violation: Based on record review, the agency failed to complete a Social History for Foster Child 2 (FC2) that addressed required elements. Findings: 1) The record for FC2 contained a document titled VA Social History. The document was blank. 2) The record for FC2 contained a document titled Comprehensive Biopsychosocial Assessment that was partially completed, but was missing key elements. 3) Staff 1 (S1) was interviewed and reported the Comprehensive Biopsychosocial Assessment is now being used to address standard 22VAC40-131-260. 4) S1 was interviewed and S1 reported that due to staff turnover some reports were lacking in FC2?s record. 5) The findings were discussed with S1 during preliminary findings review. 6) S1 acknowledged the findings.

Description:	Violation: Based on record review and interview, the agency failed to obtain a TB screening and/or assessment of freedom from communicable disease on foster child 1 (FC1) Findings: 1) An incomplete medical exam form on FC1 lacking TB screening and/or freedom from communicable disease was found in FC1?s chart. 2) No additional information could be located in the chart to substantiate why the TB screening/assessment had not been completed on FC1. 3) Staff 1 (S1) stated that the form used for FC1 was the wrong form 4) S1 was unable to provide any additional medical documentation during inspection. 5) S1 acknowledged the finding.
