Comments:

A monitoring inspection was initiated on 11/09/21 and concluded on 11/19/2021. The Homestudy Supervisor was contacted by telephone to initiate the inspection. The Homestudy Supervisor reported that there were 40 families approved since the last inspection. The inspector emailed the Homestudy Supervisor a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed six (6) adoptive home records and two (2) staff records. Background checks for providers approved since the last inspection were also reviewed. An exit interview was conducted with the Homestudy Supervisor on 11/19/21, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the agency.

Description:	Violation: Based on a review of the background checks submitted by the agency for an employee (P-1) and an interview with an agency representative (AR-1), the agency failed to obtain a criminal history background check and a search of the central registry before 30 days of employment at the agency ended. Evidence: 1) P-1 began working at the agency on 12/14/2020. The criminal history record check was dated 1/25/21 and the central registry search results were dated 2/01/21. 2) In an exit interview on 11/12/21, AR-1 acknowledged that the background checks for P-1 were dated after 30 days of employment at the agency ended.
