Technical Assistance:
Technical Assistance provided regarding Standard 22VAC40-131-180.E.2-summary of the content of interview for foster home applicants was discussed; the licensee is encouraged to be consistent in documenting compliance of this standard.

Technical Assistance provided regarding Standard 22VAC40-131-370.M.12-the licensee is encouraged to consistently utilize the effective and end dates on the electronic face sheet in provider files; documenting when household members move in and out of the provider home.

Comments:
An unannounced mandated renewal inspection was conducted to The Bair Foundation, Richmond location on December 10, 2019 from 8:30 am to 4:00 pm and on December 11, 2019 from 8:35 am to 12:10 pm. The Bair Foundation continues to provide foster care, short-term foster care and treatment foster care services. The agency reports twenty (20) children in care and twenty-five (25) approved foster homes.

During the inspection, the following activities transpired:

1. Four (4) complete foster home records were reviewed
2. Two (2) foster child records were reviewed
3. Two (2) staff records, including one (1) newly hired staff record was reviewed
4. Policy and Procedures were reviewed
5. Interviews with agency representatives were conducted
6. One Board Member Record was reviewed (background only) on 12/05/2019

There were three (3) citations for violation of the Standards for Child Placing Agencies. An exit meeting was conducted with agency representatives on December 11, 2019 at 11:30 a.m. to discuss the inspection findings. Upon receipt of the violation notice, the licensee should develop a plan of correction for each violation. The plan of correction should include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Violations:
Standard #: 22VAC40-131-230-I-2
Description: Based on the review of the record for Foster Home (FH) 2 and based on an interview with agency representatives, the licensee failed to document evidence of a written notification to the provider of their decision to suspend the home no later than one week.

Findings:
1.The record for FH2 documented an updated certificate of approval and other documentation such as a progress note indicating that the foster home was suspended; however, there was no documentation of a written notice sent to the foster home provider of the agency?s decision to suspend the home.
2.During the exit interview, AR1, AR2 and AR3 carefully reviewed the foster home record for FH2 and acknowledged the absence of a written notice of the agency?s decision to suspend the home sent no later than one week after the home was suspended.

Plan of Correction: All case managers will review suspended foster homes beginning 7/1/19 to ensure that written notification was provided to all parents. Supervisors will review all suspended homes to ensure compliance.

Standard #: 22VAC40-131-250-A
Description: Based on the review of the record for Foster Child (FC) 2 and based on an interview with agency representatives, the licensee failed to maintain written authority for placement in the child?s file.

Findings:
1.At the time of the inspection, there was no current signed placement agreement in the record for FC2.
2.During the exit interview, AR1, AR2 and AR3 carefully reviewed the record for FC2 and acknowledged that the placement agreement was absent from the record. AR2 texted the assigned case worker to inquire about the missing documentation. According to AR2, the case worker responded that the document was on her laptop but had not been uploaded to the record.

Plan of Correction: All case managers will review all cases since 7/1/19 to ensure placement agreements are in Bair Trax. Supervisors will review all cases to ensure that placement agreements are in the file.

Standard #: 22VAC40-131-280-C-8
Description: Based on the review of the record for FC1 and FC2 as well as based on an interview with agency representatives, the licensee failed to document arrangements for visits for parents in the foster home agreement.

Findings:
1.The foster home agreement located in the record for FC1 and FC2 did not contain arrangements for visits by the parent. Number 40 on the agreement states that specific visitation information is listed below however, there was no arrangements listed below, the section of the agreement was left blank.
2.During the exit interview, AR1 and AR3 reviewed the foster home agreement and acknowledged the absence of arrangements for visits by parents.

Plan of Correction: All case managers will review foster home agreements beginning 7/1/19. All
agreements will be revised to include the arrangements for visits for parents. All foster home agreements moving forward will include the arrangements for visitation with biological parents. Supervisors will review all foster home agreements for compliance.
