A monitoring inspection was initiated on August 2, 2021 and concluded on August 4, 2021. The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was 24 foster children and 32 foster home providers. There was 1 newly hired staff since the last inspection. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 1 staff record, 2 foster child records and 3 foster home provider records submitted by the facility to ensure documentation was complete. An exit interview was conducted with agency representatives on August 4, 2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or laws, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-131-370-N-1 Description: Based on the review of the records for Foster Child (FC)-1 and based on an interview with agency representatives (AR), the licensee failed to document a face sheet with the child's place of birth.
Findings:
1. The face sheet dated 06/25/2021 did not contain the place of birth. This section of the document was blank.
2. During the exit interview, AR3 stated that this was a systems error and that the information was updated the following month.Plan of Correction: Not available online. Contact Inspector for more information.
