Comments:
A monitoring inspection was initiated on 5/28/2024 and concluded on 5/31/2024. The Director was contacted by email to initiate the inspection. The Director reported that the current census was 39. A list of items required to complete the inspection was provided to the Director. Three foster child, three provider home, and four personnel records were reviewed as provided by the agency. An exit interview was conducted with on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Inspection findings will be posted to the Department?s public website within 5 days of receipt of the inspection documents. The department's inspection findings are subject to public disclosure.

The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

Description: Violation:
Based on record review, the agency failed to obtain a medical examination report addressing all elements for Foster Child 3 (FC3).

Evidence:
1) The record for FC3contained documentation of a medical examination that did not address whether the child was free of communicable disease.
3) The finding was discussed during the exit interview.
4) Saff reviewed the physical examination document for FC3 and acknowledged the findings.

Based on record review, the agency failed to conduct two face-to-face visits every month for foster child 1 (FC1), a child in treatment foster care.

Evidence:
1) The record for FC1 did not document two face-to-face visits with the child in March 2024.
2) The finding was discussed during the exit interview.
3) Staff 6 (S6) acknowledged the finding.

Description: Violation:
Based on record review and interviews, the agency failed to provide or arrange for foster child 1 (FC1) to receive recommended psychological evaluation services.

Evidence:

1) The Intake Assessment, 4/21/2023, for FC1 documented the recommendation that FC1 received a psychological evaluation.
2) The fourth quarter review and updated service plan documented FC1 ?needs to complete a psychological evaluation. Evaluations have been requested??
3) During record review, documentation of the licensee?s attempts to arrange or provide the psychological evaluation was not located.
4) The findings were discussed during the exit interview and staff 14 (S14) was interviewed. Staff reviewed the record for additional documentation.
5) The licensing inspector provided additional opportunity for staff to locate documentation and provide to the inspector.
6) Based on documentation provided, the agency made the first effort to arrange for the psychological evaluation on 12/6/2023 and a second effort on 5/29/2024.
7) The licensing inspector conducted additional interviews with Staff 5 and 6 (S5 and S6) on 5/31/2024.
8) S5 and S6 acknowledged the findings.
