Technical Assistance:
Technical assistance was provided regarding the age listed in the licensee's program description. The licensee agreed to review the program description and make the necessary updates. The definition of Independent Living Services and current Comprehensive Service Plans for a foster child 14 and over was reviewed.

Comments:
An unannounced monitoring inspection was conducted on July 15, 2019 from 9:00 am to 3:30 pm and on July 16, 2019 from 8:50 am to 11:30 am. Inspirational Youth Services, Inc. continues to provide foster care, short-term foster care and treatment foster care services. The agency reports three children in care and seven approved foster homes. During the inspection, the following activities transpired: 1. Two foster home records were reviewed. 2. One active foster child record was reviewed. 3. One discharged foster child record was reviewed. 4. Three staff records were reviewed. 5. Background checks only for two additional staff were reviewed. 6. A review of one additional foster home record associated with a Serious Incident Report was reviewed. 7. All Serious Incident Reports since the last inspection were reviewed. 8. Policy and Procedures were reviewed. 9. Interviews with agency representatives. There was one citation for violation of the Standards for Child Placing Agencies. An exit meeting was conducted with an AR (Agency Representative) on July 16, 2019 at 11:00 a. m. to discuss the inspection findings. Upon receipt of the violation notice, the licensee should develop a plan of correction for each violation. The plan of correction should include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description: Based on the review of the file for Foster Child (FC) 1, which included a service plan dated o6/17/2019, and an interview with AR1, the licensee failed to include goals and objectives that addressed specific independent living services to assist the youth in meeting the youth?s goals. The licensee also failed to include goals and objectives to address specific problem behavior, skills to be addressed, and criteria for achievement. Findings: 1. FC1 receives treatment foster care services and is older than age 14. 2. The comprehensive service plan does not document the child?s independent living needs. 3. The comprehensive service plan does not list specific independent living services to be provided to assist the child in meeting goals. 4. The file, including the comprehensive service plan did not indicate services to assist the child in meeting goals and to address specific problem behaviors, skills to be addressed and did not indicate the criteria for achievement.

Plan of Correction: Corrective Action
The Comprehensives Service Plan will be amended to: Note and document that child is older than 14. Documentation the child?s independent living needs. List specific independent living services to be provided to assist the child in meeting goals. Indicate services to assist the child in meeting goals and to address specific problem behaviors, skills to be addressed and to indicate the criteria for achievement.
Preventative Action
In the future, all comprehensive service plans will be developed to include the above.
