Comments:

An unannounced monitoring inspection was conducted on January 7, 2020 from 9:05 am to 4:00 pm. Inspirational Youth Services, Inc. continues to provide foster care, short-term foster care and treatment foster care services. The agency reports four (4) children in care and eight (8) approved foster homes. During the inspection, the following activities transpired: 1. Two (2) foster home records were reviewed. 2. One (1) active foster child record was reviewed. 3. One (1) staff records were reviewed. 4. Interviews with agency representatives were conducted. There was one (1) citation for violation of the Standards for Child Placing Agencies. An exit meeting was conducted with the Executive Director on January 7, 2020 at 3:10 p.m. to discuss the inspection findings. Upon receipt of the violation notice, the licensee should develop a plan of correction for each violation. The plan of correction should include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description:	Based on a review of the foster child (FC) record for FC1 and based on an interview with AR1, the licensee failed to document an interview in private at least one time each month for FC1. Findings: 1. According to the record, FC1 is placed in treatment foster care. 2. Based on the review of the ?Progress Notes? and the form titled ?Contacts With Child-Progress Note Checklist,? there was no documentation in the FC1?s record of a private interview with FC1 during the month of October 2019. 3. Based on an interview with AR1, at the time of the inspection, there was no documentation of a private face to face session with FC1.
Plan of Correction:	Corrective Action: A private interview was conducted and the record will be amended to document the interview. Preventive Action: In the future, a private interview with be conducted monthly with each Foster Child. Staff Responsible: Case Manager
