22VAC40-131-140.G: Use of Case worker trainees and documentation of the written training program.

Comments:

A monitoring inspection was initiated on August 5, 2021 and concluded on August 10, 2021. The Program Director/Executive Director was contacted by telephone to initiate the inspection. The Program Director/Executive Director reported that the current census was 4 foster children, 3 active foster home providers and 2 newly hired staff since the last inspection. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 1 foster child record, 1 provider home record, and 2 employee records submitted by the facility to ensure documentation was complete. An exit interview was conducted with agency representative on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or laws, and violations were documented on the violation notice issued to the facility.

Description:	Based on the review of the documents submitted for Foster Child (FC)-1 and based on an interview with an agency representative, the licensee failed to maintain documentation of entries in the case file with the signature of the individual performing the service. Findings: 1.According to the document titled ?Progress Note,? S1 is the individual performing services to FC1 on 05/24/2021. This documentation was not singed by S1. 2.According to the document titled ?Progress Note,? S2 is the individual performing services to FC1 on 07/06/2021 and 07/12/2021. These documentations were not singed by S2. 3.During the exit interview, AR-1 acknowledged that the Progress note for 05/24/2021 was completed by S1 and not signed by S1. 4.During the exit interview, AR-1 acknowledged that the Progress notes completed on 07/06/2021 and 7/12/2021 was completed by S2 and not signed by S2.
