Comments:

An unannounced mandated monitoring inspection was conducted on January 18, 2022 from 09:45 am to 4:00pm and January 19, 2022 from 9:00 am to 12:20 pm at the Mechanicsville location. The following is a list of activities for this inspection: 1.Two (2) foster children records reviewed 2.One (1) provider home record reviewed 3.Three (3) staff records were reviewed 4.A physical plant inspection was completed Staff were available for the preliminary findings/exit interview which commenced at approximately 11:45 a. m. on January 19, 2022 at the Mechanicsville location. An acknowledgement form was signed and left with the licensee at this time. There were two (2) citations for violations of Standards for Licensed Child-Placing Agencies. Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non- compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on the review of the record for Foster Child (FC)-1 and based on an interview with an Agency Representative (AR), the licensee failed to ensure that the written documentation contained the signature of the individual performing the service. Findings: 1. There was a progress note located in the record that documented services which were independently performed by Staff (S)-1. This progress note contained the signature of AR-2 only with no signature by S-1. 2. There were progress notes documenting services completed by S-1 & AR-2 (jointly) which did not contain the signature of S-1. 3. During the exit interview, AR-2 acknowledged that the signature of the person performing the services as referenced above were missing from the documentation.

Description:	Based on the review of the record for Staff (S)-2 and based on an interview with an Agency Representative (AR)-2, the licensee failed to obtain a central registry and criminal history check before three years since the dates of the most recent central registry finding and most recent criminal history record check report for S-2. Findings: 1. The date of the last central registry check located in the record for S-2 is dated 12/15/2015. 2. The date of the last criminal background check located in the record for S-2 is dated 10/01/2018. 3. During the exit interview, AR-2 acknowledged the absence of a central registry and criminal background check for S-2 before three years since the dates of the last reports.
