Comments:

Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 7/19/2022 from 9:45a.m. to 12:45 p.m. and 7/20/2022 from 9:00 to 9:45 a.m. The Licensing Inspector met with the Executive Director at the site. Number of approved provider homes: (2) Number of children in care: (2) The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (1). Number of provider home records reviewed: (1) Number of staff records reviewed: (0) as there were no new staff hired since the last inspection. Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. There were no changes made to the policies or procedures since the last inspection. The evidence gathered during the inspection determined (2) violations with applicable standard(s) or law. Description:	Violation: Based on a review of the record provided for Foster Home (FH) FH1, Individual #1, the licensee failed to obtain and document at least one non-relative reference out of the minimum three references for the applicant. Findings: Individual #1 is an adult household applicant and the three references obtained were provided by relatives. The licensee failed to obtain at least one reference from a nonrelative. During the exit interview AR-1 reviewed the record and acknowledged the absence of the documentation.

Standard #:	22VAC40-131-290-A-1
Description:	Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to document vision examination for the individual. Findings: 1. Based on the review of the physical examination report dated 6/17/2022 for FC1, there was no documentation of a completed vision examination report for FC1. 2. During the exit interview AR-1 reviewed the record and acknowledged the absence of the documentation of a vision examination.
