Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 11/30//2023 and from 10:00 to 12:00 pm. The licensing inspector attempted an inspection on 11/29/2023 and the licensee was not available at the location or via telephone. Number of children in care: 0 Number of approved provider homes: 1 Number of provider home records reviewed: 1 Number of staff records reviewed: 3 Observations by the licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. The evidence gathered during the inspection determined (1) violation with applicable standard(s) or law. Description:	Violation: Based on the record review for FP1, the agency failed to monitor the home at least one time every 90 days Findings: 1. A review of the contact sheet for FP1 showed the last contact was 7/24/2023. There was no follow-up contact monitoring note with the foster parent as of 11/30/2023. 2. The findings were discussed during the preliminary findings and exit.
