Comments:

A monitoring inspection was initiated on 08/24/2021 and concluded on 09/02/2021 (The Inspector was summonsed to jury duty that created a delay). The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was (45) forty-five active foster child placements, (57) fifty-seven active foster home providers and (1) one newly hired staff since the last inspection. The Inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The Inspector reviewed (4) four complete foster child records, (6) six foster home provider records and (1) one employee record. An exit interview was conducted with agency representatives on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on the review of the file for Foster Child (FC)-2 and based on an interview with Agency Representatives (AR), the licensee failed to document the place of birth on the face sheet for FC-2. Findings: 1.The place of birth section of the face sheet for FC-2 is blank. 2.The birth certificate for FC-2 is located in the file and dated 06/03/2021. 3.During the exit interview, AR-3 confirmed the receipt of the birth certificate with a received date of 07/27/2021. 4.During the exit interview, AR-3 and AR-1 acknowledged that the face sheet was not updated with the place of birth for FC-2.
